He drinks occasional alcohol and smokes 20 cigarettes a day, which he has done since the age of 16. His symptoms had started one week previously when he noticed a burning sensation when passing urine as well as some low abdominal pain. After using some sachets and cranberry juice capsules that his wife had bought from the chemist, the suprapubic pain eased. A week later he developed left-sided back ache which became worse and started to move from his ‘back to his groin, He also experienced short-lived episodes of ‘chills’ and uncontrollable shaking. It was at this point that he presented to his General Practitioner. From further questioning it was evident that for approximately 3 months prior to his current symptoms he had noticed blood in his urinary stream on several ‘occasions but because it had caused him no discomfort, he had failed to mention it to anyone. Leaming Objectives 1. Describe the anatomy and function of the Urinary tract, both gross and histological 2. List the key features in taking a urological history 3. Describe symptoms associated with a urinary tract infection 4, List the key features of a focussed history of haematuria 5. Describe the significant causes of haematuria 6. Explain the likely cause underlying the patient’s symptoms: Haematuria Pattern of pain (nature and sight) Cloudy urine Rigors boca 7. Provide a differential diagnosis that best fits the patient’s symptoms, as well as alternatives placed in order of likelihood 1/31
